While we strongly encourage attendance at one of our live seminar presentations, we also offer the opportunity to view an online seminar.

Once you have completed viewing the online seminar (it is approximately 50 minutes in length), please complete the following: click here to send us an email confirming you have watched the seminar, there will be a survey within the email, please take the time to answer those questions; then click this link to complete your online health questionnaire.

To register for one of our live seminars, please click on the date of the seminar that best fits your schedule and complete the short registration process. Then, to complete your paperwork online, please click on this link. Alternately, you may download a paper version of the New Patient Paperwork to complete and bring with you to the seminar. You may also fax this information to us at (615) 342-1814. When registering online, please add your insurance information (insurance company name, group number, ID or member number and phone number) in the comments box.

All seminars are held on the Centennial Medical Center campus in either the Tace Building (Center for the Treatment of Obesity) or the Medical Office Plaza auditorium.

Tueday seminars are held in the Tace Building located at 2200 Murphy Avenue. From Charlotte Avenue, turn onto 22nd Avenue, turn right onto Murphy Avenue, then turn right in to the parking lot. The Tace building is next door to the parking lot. Please proceed up the ramp and enter through the front doors of the Tace Building and in to the waiting room.

Saturday seminars are held in the Medical Office Plaza Auditorium located at 2201 Murphy Avenue. From Charlotte Avenue, turn onto 22nd Avenue, turn right onto Patterson Street, free parking is available on the left (signs will be posted, you may also park in the area designated as physician's parking immediately outside of the entrance). The entrance closest to the auditorium is on the corner of 22nd and Patterson (signs will be posted).
